Abstract
This poem highlights the relationship-based nature of an integrated nursing and mental health home visiting model designed to support vulnerable, expectant young parents. It chronicles a young woman's internal experience as she contemplates accepting intervention from the nurse and social worker team. Finally, the poem illuminates the parallel process of the nurse and social worker holding the mother's hopes, dreams, trauma, fears, and best intentions as a parent, as she holds her child which is reflected in the accompanying illustration.
